Formerly known as the Living Will, the Advanced Care Plan was created by a new Tennessee Law in 2004 and is far more comprehensive than a Living Will.  It allows you to communicate your healthcare preferences after you have lost the capacity to make or communicate your own decisions. Even if you already have a Living Will, you might want to use this to update your information.   It’s also a good idea to notify persons to whom you have distributed your Living Will and give them a copy of your new Advanced Care Plan.  To obtain an Advance Care Plan, click here.
